A tired appearance that persists however much you sleep. Dark circles have four quite different causes, and creams fail most often simply because they were never aimed at the right one.

Dark circles look like one problem but they are four. Pigmentation is excess melanin in the skin itself, common in Indian skin and often inherited. Vascular circles are the bluish look of blood vessels showing through skin that is naturally very thin. Structural circles are a shadow cast by a hollow between the lower eyelid and the cheek. Textural circles come from crepey, sun damaged or loose skin scattering light unevenly.
Most men have a combination, usually with one type dominating. That is why generic under eye creams disappoint so often: a cream aimed at pigment does nothing for a hollow, and nothing at all improves a shadow that is caused by bone structure.
Two simple tests in clinic separate them. Stretching the skin gently makes pigment stay and vascular colour fade. Changing the direction of the light makes a structural shadow move while pigment does not.
Under eye darkening is very common in Indian skin, where pigmentation is the leading cause and frequently runs in families. It often appears in the twenties rather than in later life, which surprises men who assume it is purely an ageing change.
Men tend to seek help when it starts to affect how they are perceived at work, because a permanently tired appearance gets commented on. That is a perfectly reasonable reason to have it assessed.

Assessment is worthwhile mainly to identify the type before spending money on products aimed at the wrong one.
Under eye darkening is not urgent. Sudden swelling of one eyelid with pain, redness, fever or change in vision needs same day medical assessment, as does new bruising around both eyes after a head injury.
The assessment is quick and largely mechanical. Identifying the dominant type is the whole point of the visit.
The area is examined under overhead and front lighting. A shadow that changes with the direction of the light is structural, while pigmentation looks the same whichever way the light falls.
Gently stretching the skin makes vascular colour fade and leaves true pigment unchanged. This simple manoeuvre separates the two commonest types within seconds.
Skin thickness, fine lines, puffiness, fat position and the groove between eyelid and cheek are assessed, since these decide whether resurfacing, volume or surgery is appropriate.
Iron studies, thyroid function and an allergy history where symptoms suggest it. These are inexpensive and occasionally reveal a contributor that treatment of the skin alone would never fix.
Pigmentation responds gradually over months to topical treatment and gentle resurfacing, and it needs sun protection to hold the result. Structural hollowing responds quickly and dramatically to volume replacement, often within a single appointment. Vascular circles improve modestly at best, because thin skin cannot be thickened greatly.
Where loose or crepey skin is the main issue, resurfacing and, in selected men, eyelid surgery gives the biggest change, and these are discussed within male facial rejuvenation.
Topical treatment is a modest ongoing cost. Courses of peels or gentle resurfacing are priced per course rather than per session. Volume replacement is priced by the amount used and typically needs repeating after a year or more. Eyelid surgery, where it is appropriate, is a one time figure covering surgeon, theatre and follow up. Everything is quoted after assessment.
Treatment is matched to the type. Combining two approaches is common because most men have more than one cause.

Cool compresses reduce puffiness temporarily and are harmless. Lemon juice, toothpaste and similar remedies can irritate very thin eyelid skin and sometimes leave it darker than before. Sun protection is the one home measure with genuine evidence behind it.
In experienced hands it is generally safe and the results can be excellent, but the under eye area is unforgiving. Poor technique causes lumps, puffiness and a bluish tint. It should be performed only by someone who treats this area regularly.
Volume replacement typically lasts a year or longer before it needs repeating. Pigment improvement holds as long as sun protection and maintenance treatment continue. Surgical correction of loose eyelid skin is long lasting, though the skin continues to age normally afterwards.
Yes, and it is underestimated. Persistent allergic inflammation congests the small vessels under the eye and prompts rubbing, which then darkens the skin further. Controlling the allergy and stopping the rubbing often produces visible improvement without any cosmetic treatment.
